Here is the thing that decides this purchase, and neither vendor puts it on the front of the brochure: Lexicomp and Micromedex run their IV compatibility tools on the same underlying database. Both incorporate Trissel's 2 Clinical Pharmaceutics Database by Lawrence A. Trissel. Wolters Kluwer says so in a footnote on its own product page, and Merative says so in the first line of its own product description.
So if you are a pharmacy director holding two quotes and trying to work out which vendor has better compatibility data, the honest answer is that the source data is the same and the difference is in the layer each vendor builds on top of it. That reframes the decision entirely, and it is worth an hour of your time before you sign anything.
Both tools are Trissel's underneath
Lawrence Trissel's Handbook on Injectable Drugs is the reference standard for parenteral compatibility and stability, and the electronic version of that work, Trissel's 2 Clinical Pharmaceutics Database, is licensed to more than one vendor. Wolters Kluwer's page for its Trissel's IV Compatibility tool carries the footnote "Incorporates the Trissel's 2 Clinical Pharmaceutics Database by Lawrence A. Trissel," and describes its own contribution as having "built on the trusted compatibility database from Lawrence A. Trissel" and redesigned the module with practicing pharmacists and usability experts. Merative's page for Micromedex IV Compatibility says the tool is "powered by Trissel's IV compatibility tool (Trissel's 2 Clinical Pharmaceutics Database)" and "continuously updated according to robust Micromedex editorial processes."
Read those two sentences next to each other and the shape of the market is clear. You are not choosing between two research programs. You are choosing between two editorial teams, two interfaces and two commercial relationships, all sitting on one body of published compatibility work.
The same data also appears inside a third Wolters Kluwer product. The company's own support material tells users how to reach compatibility information "within UpToDate Lexidrug and within Facts and Comparisons," which means a pharmacy that licenses both of those products is paying two subscriptions to reach one database. That is worth checking before you add a third vendor to the pile.
What each vendor publishes about coverage
The counts differ, and the difference is real rather than marketing. Here is what each vendor states on its own site, read in September 2026.

Micromedex publishes the larger numbers on both counts. Some of that gap is definitional: Merative counts parenteral drugs and solutions together and calls out chemotherapy agents specifically, while Wolters Kluwer's 850 figure covers "drugs and solutions" as one bucket too but leads with the reference count instead. Do not treat the difference as proof that one product will answer a question the other cannot. Treat it as a prompt to test both against the twenty combinations your pharmacy actually prepares.
Is IV compatibility in Lexicomp the same as Micromedex?
The source data is the same and the editorial layer is not. Both incorporate Trissel's 2 Clinical Pharmaceutics Database, so the underlying compatibility studies are identical. What differs is how each vendor curates and presents them: Micromedex separates results by Y-site, syringe and admixture and surfaces study conditions, concentration and manufacturer, while Lexidrug leads with quick Y-site filters and folds compatibility into the same interface as the rest of its drug monographs.
That last point is the one that usually decides it in practice. If your pharmacists already live inside one of these platforms all day, adding the compatibility module to the platform they are already in is worth more than a few hundred extra entries in a database they will reach for twice a week.
Neither vendor publishes a price, and what to do about it
This is a quoted category. Wolters Kluwer publishes no figure for the Trissel's module, and Merative publishes no figure for Micromedex IV Compatibility. Both route you to a sales conversation, and Wolters Kluwer splits that conversation two ways: one path for individual clinicians, independent community pharmacies of one to ten stores and independent medical practices, and another for hospitals and health systems where the call is with a Lexidrug expert.
Which path you land in matters more than the list price you never see. The institutional motion has none, and the number depends on bed count, site count, EHR integration depth and how many other content sets you are buying at the same time.
Three things make an institutional quote go better. Ask for the compatibility module priced separately as well as bundled, so you can see what the bundle is actually buying you. Ask whether the module is included in any content set you already license, because in the Wolters Kluwer family it often is. And get the whole thing on paper before it reaches finance, since a quoted software license with no public list price is exactly the sort of spend that stalls when nobody can produce a comparison, so it helps to have the purchase order raised against a documented quote rather than an email thread.
Micromedex IV compatibility free: what changed
People search for this because there used to be a standalone Micromedex IV Compatibility app, and it was free to subscribers of the online product. Merative folded that functionality into a single Micromedex app that carries drug reference, interactions, IV compatibility and calculators together. The compatibility feature is still there, still available on mobile, and still requires an underlying subscription. There is no free public IV compatibility checker from either vendor, and there should not be: this is the sort of data where a stale or incomplete answer harms a patient.
Free compatibility charts circulate widely, usually a manufacturer's Y-site table or a departmental PDF someone scanned years ago. They are fine as a memory aid for a combination you already know and unsafe as a primary source, because they do not carry the study conditions, the concentrations tested or the date the entry was last reviewed. If your department is currently running on a laminated chart, that is the actual finding from this exercise, and it matters more than which vendor you pick.
Which one to buy, by setting
Hospital and health system. Buy the one your pharmacists are already in. If you hold Micromedex for monographs, the compatibility module belongs there; if you hold Lexidrug, the same logic applies in reverse. The coverage gap between the two is not large enough to justify introducing a second login and a second vendor relationship for this alone. Where Micromedex earns a genuine edge is oncology infusion and TPN work, because it names chemotherapy agents and total parenteral nutrition explicitly in its coverage.
Home and specialty infusion pharmacy. This is where compatibility is the whole job rather than one module among many, and where the syringe and admixture views matter as much as Y-site. Micromedex's explicit split across Y-site, syringe and admixture maps more directly onto what a home infusion pharmacist prepares. Price it against a Lexidrug package that includes compatibility, because the bundled route is sometimes cheaper even when the standalone module is not.
Ambulatory infusion centers and small practices. You are in the individual and small business lane, where Wolters Kluwer publishes a price and Merative does not. That alone tends to decide it, not because the product is better but because a $29.99 a month anchor is something you can budget against without a procurement cycle.
What a compatibility database still will not do
It answers the question you bring it. It does not look at the patient's list and tell you that the combination you are about to hang is a problem, and it does not know that the same patient's renal function moved yesterday. Compatibility is a physical chemistry question about what happens in the line; the pharmacology question about what happens in the patient is separate, and neither Trissel's nor anything built on it addresses it.
