What drugs should not be taken with NSAIDs?

The combinations most consistently avoided are NSAIDs with an anticoagulant, NSAIDs with lithium, NSAIDs in a patient already taking an ACE inhibitor or ARB together with a diuretic, and NSAIDs in decompensated heart failure or advanced chronic kidney disease. A second NSAID, including over-the-counter ibuprofen or naproxen taken on top of a prescription, belongs on the same list, since there is no analgesic gain above the ceiling dose and the toxicity is additive.

Everything else on this page is a monitoring question rather than a prohibition. An SSRI plus an NSAID is common and often reasonable with gastroprotection. Low dose weekly methotrexate with an NSAID is standard rheumatology practice. Digoxin and NSAIDs coexist with level checks. The distinction that matters clinically is whether the interaction has a monitoring plan attached to it, not whether the checker flagged it.

What is the triple whammy in medicine?

The triple whammy is the combination of an NSAID, an ACE inhibitor or ARB, and a diuretic. Each drug removes a different part of the kidney's ability to defend glomerular filtration when perfusion drops. The NSAID takes away the prostaglandins that dilate the afferent arteriole, the ACE inhibitor or ARB takes away the angiotensin II that constricts the efferent arteriole, and the diuretic takes away the circulating volume. In a well patient nothing happens. Add gastroenteritis, hot weather, or a poor oral intake for two days and the filtration pressure collapses.

What makes it a clinical problem rather than a textbook one is that the three drugs are usually prescribed by different people at different times, and the NSAID is often bought off a shelf. It is the interaction most worth checking deliberately in older primary care patients, and the one where sick-day guidance (hold the ACE inhibitor, the diuretic and the NSAID during an acute illness with vomiting or diarrhea) does the most good.

Why does ibuprofen interfere with low dose aspirin?

Aspirin works for cardioprotection because it acetylates a serine residue on platelet COX-1 irreversibly, and platelets cannot make new enzyme. Ibuprofen binds the same channel reversibly. If ibuprofen is occupying the site when the aspirin arrives, the aspirin cannot acetylate it, the block wears off with the ibuprofen, and the patient gets analgesia without the antiplatelet effect they are actually taking aspirin for.

The FDA has advised counseling patients on timing: with immediate-release low dose aspirin, take ibuprofen at least 8 hours before or at least 30 minutes after the aspirin dose. The agency also noted that occasional ibuprofen use is unlikely to undermine cardioprotection, because the effect of daily aspirin persists. The interaction matters most for a patient taking scheduled ibuprofen for arthritis while relying on aspirin for secondary prevention, and enteric-coated aspirin complicates the timing further because absorption is delayed.

Can you take NSAIDs with methotrexate?

It depends entirely on the methotrexate dose, and this is where general interaction warnings mislead. At the high doses used in oncology, NSAIDs are avoided, because delayed methotrexate clearance in that setting produces mucositis, myelosuppression and renal injury. At the low weekly doses used for rheumatoid arthritis and psoriasis, methotrexate and NSAIDs are co-prescribed routinely and have been for decades.

The condition attached to that routine use is monitoring rather than indifference. Renal function, blood counts and liver enzymes are followed, and the risk rises if renal function falls, if the patient is older, or if a second nephrotoxic drug or dehydration enters the picture. The failure mode is not the planned combination in a rheumatology clinic. It is the patient on weekly methotrexate who starts taking naproxen twice daily for a back strain during a diarrheal illness, with nobody following anything.

Do all NSAIDs have the same interaction profile?

No, though the differences are smaller than patients assume. The renal and blood pressure effects are close to a class property, because they follow from prostaglandin inhibition itself, and celecoxib is not exempt from them. Antiplatelet effect is where the class genuinely splits: COX-2 selective agents such as celecoxib have far less effect on platelet aggregation, which is why they are sometimes chosen when bleeding risk dominates, while ketorolac sits at the opposite end with the strongest bleeding signal in the class.

Cardiovascular thrombotic risk carries a boxed warning across the class, and comparative data have generally placed naproxen at the lower end of that signal. Route matters too: topical diclofenac produces a small fraction of the systemic exposure of oral dosing, which is why it appears as a substitute throughout this page for patients on anticoagulants or with reduced renal function, although its labeling still carries the class warnings. It is one of the combinations most often avoided. Ibuprofen adds platelet inhibition and direct gastric mucosal injury to whatever the anticoagulant is already doing, so gastrointestinal bleeding risk rises sharply with warfarin and with the direct oral anticoagulants alike. With warfarin the INR frequently does not move, so monitoring gives false reassurance. Acetaminophen or a topical NSAID is the usual substitute, with gastroprotection when an oral NSAID cannot be avoided.
None of them is kidney-neutral, because the renal effect follows from prostaglandin inhibition itself and applies to COX-2 selective agents as well. What reduces risk is exposure rather than the choice of agent: the lowest effective dose, the shortest course, adequate hydration, and avoiding the combination with an ACE inhibitor or ARB plus a diuretic.
