In short

Isabel DDx Companion costs $149 a year for the Standard plan and $199 a year for Premium, which adds ChatGPT-powered Diagnosis Compare and Diagnosis Explained tools that run on desktop only and in English. Students and residents pay $49 a year with a discount code requested from Isabel, and every plan starts with a 30-day free trial. Institutions get a quote. Premium is worth the extra $50 only for clinicians who work up cases at a desktop; on a phone you get the Standard experience either way.

Isabel DDx Companion, the differential diagnosis generator from Isabel Healthcare, sells two individual plans in the US: Standard at $149 a year and Premium at $199 a year. Students and residents pay $49 a year with a discount code, and every plan starts with a 30-day free trial. We read the vendor's individual pricing page and its launch post for the ChatGPT features in October 2026, and the plan contents below come from those two pages.

The question most clinicians bring to that page is narrower than "how much". It is whether the extra $50 for Premium buys anything they will use, and whether a free AI tool now does the same job. Both have a straight answer.

One detail trips people up. Older descriptions of the Premium plan, which still appear in some search snippets, say it bundled access to 5MinuteConsult.com. The current pricing page does not mention that; Premium now means the ChatGPT integration. If you were buying Premium for the 5 Minute Clinical Consult, read our 5 Minute Clinical Consult app guide first, because that reference is sold separately at $99.99 a year in the app.

Is Isabel Premium worth the extra $50?

Only if you work at a desktop. Isabel's own launch post, dated October 22, 2025, says the ChatGPT features run on desktop only and in English. On a phone in the corridor you get the Standard experience whichever plan you paid for.

What the extra $50 buys is speed in the narrowing step. You pick two to five diagnoses from Isabel's list and get a side-by-side table of distinguishing symptoms, the tests worth ordering, prognosis and treatment. Isabel is careful to say the LLM does not build the differential; it works from the list Isabel's own engine produced, which keeps the starting point consistent. For a hospitalist or outpatient internist working up an unclear presentation at a workstation, that table can save ten minutes of reading. For an emergency physician who does most of the thinking on a phone, Standard is the better buy.

Is there a student discount for Isabel DDx?

Yes. Students and residents pay $49 a year, against $149 for Standard. The discount is not a checkout button: you send your details through Isabel's form and they email a discount code. Isabel positions the tool as a way to practice clinical reasoning, and the student rate is the cheapest paid diagnostic aid we have priced. Students building a study routine around cases often pair it with a way to turn their clinical reasoning notes into practice questions, since generating a differential and recalling it under exam pressure are different skills.

Isabel DDx vs the free AI tools

This is the comparison that decides most purchases in 2026. OpenEvidence is free for verified US healthcare professionals, and OpenAI's ChatGPT for Clinicians is free at launch for NPI-verified physicians, NPs, PAs and pharmacists. Both will discuss a differential if you describe the case. Glass Health sells AI clinical decision support with a free Lite tier and paid plans at $20, $90 and $200 a month, which we break down plan by plan, with the yearly rates, in Glass Health pricing.

ToolUS price for one clinicianHow it builds a differential
Isabel DDx Companion$149 or $199 a year; $49 students and residentsIsabel's own engine (in development since 1999) produces the list; Premium adds LLM comparison on top
Glass HealthLite $0; Starter $20, Pro $90, Max $200 a monthLLM-based decision support bundled with an ambient scribe; Max adds EHR integration
OpenEvidenceFree for verified US cliniciansAnswers questions with citations from licensed journals; you prompt it with the case
ChatGPT for CliniciansFree at launch for verified cliniciansGeneral model with cited clinical search; individual BAA available
5 Minute Clinical Consult app$99.99 a yearTopic reference with diagnostic algorithms and Diagnosaurus; you navigate by condition
DynaMedex$599 a yearPoint-of-care topic reference you search by condition, with drug content included

Isabel's argument for itself is consistency: the same inputs give the same list every time, which a general chat model does not promise. That matters if you document why a diagnosis was considered, or if you teach. The free tools win on breadth. They answer the follow-up question about management in the same window. If you are deciding between OpenEvidence and a paid reference more broadly, our OpenEvidence cost page and the ChatGPT for doctors guide cover what each free tier really includes.

Who should buy which Isabel plan

  • Emergency physician or urgent care clinician: Standard at $149. You work on a phone and in short bursts; the ChatGPT tools would sit unused.
  • Hospitalist or outpatient internist: Premium at $199 if you do your workups at a desktop and want the compare table. Run the 30-day trial on three real cases before deciding.
  • Resident or student: the $49 rate. Ask for the code before the trial ends so you do not pay the full price by default.
  • Clinician already using OpenEvidence daily: try Isabel's trial only if you want a repeatable list to check your own thinking against. Many will find the free tool enough.
  • Practice owner buying for a group: ask Isabel for an institutional quote; individual plans are priced per person.

Where a differential tool stops

Isabel ends at the diagnosis. Once you settle on, say, pyelonephritis in a 78 year old on apixaban with a creatinine clearance of 27, the next decision is the prescription, and that is where a different kind of tool earns its keep. The antibiotic choice has to clear the renal threshold, the interaction with the anticoagulant and any documented allergy.

That is the step Prescriber.io covers. You describe the patient once, name the drug, and one check returns interactions, contraindications, allergy conflicts and renal or hepatic dose adjustments, each with a cited source for you to verify and sign. It is $24 per clinician a month billed annually. The drug dosage calculator app page shows how the dosing side works on a real case, and the drug contraindication checker covers the condition and allergy screens. Isabel tells you what the patient has; we help you check what you are about to give them.

Frequently asked questions

How much does Isabel DDx cost?

Isabel DDx Companion costs $149 a year for an individual Standard subscription and $199 a year for Premium, read on Isabel Healthcare's pricing page in October 2026. Students and residents pay $49 a year with a discount code, and there is a 30-day free trial. Hospital and program licenses are priced by quote.

What is the difference between Isabel Standard and Premium?

Standard gives you Isabel's differential diagnosis list with each diagnosis linked to Merck Manual, PubMed and other sources. Premium adds ChatGPT-powered tools: Diagnosis Compare puts two to five diagnoses side by side on symptoms, tests, prognosis and treatment, and Diagnosis Explained summarizes each one. The ChatGPT tools are desktop only.

Is there a free trial of Isabel DDx?

Yes. Isabel Healthcare offers a 30-day free trial of Isabel DDx Companion. Use it on real cases at the place you actually work, phone or desktop, because the Premium ChatGPT features only run on desktop and that decides whether the extra $50 a year is worth paying.

Is there an Isabel DDx app?

Isabel works in the browser on desktop, tablet and phone, and its pricing page says mobile access is included with no app required. The Premium ChatGPT compare and explain tools are currently desktop only, so on a phone both plans behave like Standard.

Does Isabel Premium still include 5MinuteConsult?

Not according to the current pricing page. Older descriptions said Premium bundled 5MinuteConsult.com access, but in October 2026 Premium is described as the plan with the ChatGPT integration. The 5 Minute Clinical Consult is sold separately, at $99.99 a year in the Unbound Medicine app.

Is Isabel better than OpenEvidence for differential diagnosis?

They do different jobs. Isabel generates a differential from the same structured inputs every time, which suits documentation and teaching. OpenEvidence is free for verified US clinicians and answers case questions with journal citations, including management. Many clinicians start with the free tool and pay for Isabel only if they want a repeatable list.
