Prescriber.io

For clinicians without a hospital login

UpToDate drug interactions checker access, cost and alternatives

The UpToDate drug interaction checker is excellent and it belongs to whoever pays for the login. If that is no longer your employer, here is what the tool does, what it costs to get back, and what else checks a medication list.

Or run the list here. Prescriber.io checks it against the patient, not just against itself.

Decision-support check

Press Run check for the interaction, contraindication, dosing and alternatives on this pair.

That was your demo check, run on the same references the desk uses. The desk adds renal dosing against an estimated clearance, hepatic review, unlimited checks, saved history and your sign-off on every card.

Not in the on-page check

The on-page check covers five prescribing scenarios and never invents clinical output, so it will not produce a card for this pair.

The full Prescriber.io desk checks any regimen against interactions, contraindications, and renal or hepatic dosing, with cited sources for you to verify.

The short answer

Does UpToDate have a drug interaction checker, and what does it cost?

Yes. UpToDate includes a Drug Interactions tool, built on UpToDate Lexidrug (formerly Lexicomp), that analyzes drug-drug, herb-drug and herb-herb combinations and rates each pair from A (no known interaction) to X (avoid combination). It comes as part of UpToDate (the app listing names drug reference and interactions among its features) and is not sold separately. UpToDate publishes no personal price, so the cheapest published route to the same interaction data is the Lexidrug app at $29.99 a month. Prescriber.io is a different kind of check: it screens the list against the patient, with contraindications and renal or hepatic dosing, for $24 per clinician a month billed annually.

How the tool works

How to use the UpToDate drug interactions tool

Open UpToDate and click Drug Interactions under the search bar. The same link sits in the menu at the top right of every page. Type each drug or herb to build the list, then click Analyze.

Every interacting pair comes back with a letter. Click a pair and you get the interaction monograph: the rating, a summary of what happens, and patient management advice. You can filter the result by rating and print it for the chart or the patient.

D and X are the ratings that should stop you before you sign. C is the long tail, and on a list of twelve drugs there can be a lot of it. The data underneath is UpToDate Lexidrug, which is why the ratings match what a hospital pharmacist sees in the Lexicomp drug interaction checker.

Strengths and limits

What UpToDate drug interactions cover and where the tool stops

Three things it does better than almost anything else, and three it was never built to do.

Drug, herb and herb pairs

The tool analyzes drug-drug, herb-drug and herb-herb combinations. Add the whole list and it returns every interacting pair it knows about.

A monograph behind every rating

Each pair opens an interaction monograph with the risk rating, a summary and patient management notes. This depth is the reason pharmacists trust it.

Filter and print

You can filter the result by rating and print it, which helps when a long list returns twenty C ratings and one D that matters.

It does not know the patient

You type drug names. Kidney function, liver disease, age and allergies are not part of the analysis, so the same list returns the same result for every patient.

Contraindications live elsewhere

Whether a drug is unsuitable for a condition is in the drug monograph. You look that up separately for each drug on the list.

Access follows the login

Most people who use it never paid for it. Change jobs, graduate or open a practice and the tool goes with the badge.

Same list, different patient

Why a drug-name checker and a patient check give different answers

Take a list most primary care clinicians see weekly: apixaban, metformin, lisinopril, and you are about to add trimethoprim-sulfamethoxazole for a urinary tract infection. Any good interaction checker flags the antibiotic with the ACE inhibitor for hyperkalemia.

Now make the patient 81 with a creatinine clearance of 28. The hyperkalemia risk is no longer a footnote, the antibiotic needs a renal dose adjustment, and metformin is sitting close to the kidney threshold where its labeling says stop. None of that is an interaction between two drug names, so a name-based tool cannot raise it.

That gap is what a patient-aware check closes. Our drug contraindication checker page covers the condition screens, and the renal dosing calculator shows the kidney thresholds.

Access and price

UpToDate interaction checker access routes and what each costs

Seven ways a US clinician gets a professional interaction check, with the price each vendor publishes.

Getting the UpToDate checker without a hospital login

Start with what you may still have: a hospital affiliation or a teaching appointment sometimes carries remote UpToDate access, so ask the library before you pay. If nothing applies, a personal subscription brings the tool back along with everything else in UpToDate. Our page on UpToDate subscription cost explains why no figure is published and how the store flow works, and the UpToDate discount guide lists the society rates.

If the checker is the only part you miss, the Lexidrug app is the cleaner purchase. It is the same interaction database with fuller monographs at $29.99 a month, and the Lexidrug subscription page covers the trial and what is in the package. The UpToDate vs Lexicomp comparison sets the two side by side.

Where UpToDate wins

On depth, plainly. The interaction monographs are written and maintained by a large pharmacist editorial team, the herb coverage is real, and the tool sits one click from the clinical topic you were already reading. If your institution pays, use it. A free checker such as the one behind our Medscape drug interaction checker review is a reasonable second opinion, and a patient-aware check earns its place when most of your risk comes from kidneys, age and conditions more than from drug pairs.

How Prescriber.io works

Check a medication list against the patient in four steps

  1. 1

    Paste the medication list

One drug per line, brand or generic, the way it sits in the chart. Add the drug you are about to prescribe.

  2. 2

    Add the patient

Age, sex, weight, serum creatinine and liver function. This is the part a drug-name checker never asks for.

  3. 3

    Read one card

Interactions, contraindications and renal or hepatic dose flags sorted by severity, each with the mechanism and a cited source.

  4. 4

    Verify and sign

You check the flags that matter against current labeling and make the call. The card supports the decision; it does not make it.

It runs in the browser on a phone, tablet or desktop. The drug interaction checker page shows the full card, and the dose side has its own drug dosage calculator app page.

Who pays for a checker

Who ends up buying an interaction checker of their own

Inside a hospital the check is done twice before a dose reaches the patient, once by the EHR and once by a pharmacist with a licensed database. Outside it, the prescriber is often the only check there is.

Hospital pharmacist checking a drug reference at a pharmacy workstation

Clinicians who just left a hospital

UpToDate came with the job. In a private or independent practice the interaction check has to be bought, per clinician.

NPs and PAs in outpatient practice

A lightweight EHR with noisy alerts and no pharmacist down the hall. The check has to be right before the prescription leaves.

Small practices buying for a team

Three to thirty prescribers who need the same check and one invoice, without an enterprise sales cycle.

Telehealth and urgent care

New patient, an incomplete list and ten minutes. Interactions and renal dosing need to arrive in one pass.

Every plan is billed per prescribing clinician. Full detail on the pricing page.

Straight answers

Questions clinicians ask about UpToDate drug interactions

Does UpToDate have a drug interaction checker?

Yes. UpToDate includes a Drug Interactions tool that analyzes drug-drug, herb-drug and herb-herb combinations and rates each pair from A (no known interaction) to X (avoid combination). You open it from the UpToDate homepage under the search bar or from the menu, add the drugs and click Analyze. The interaction data comes from UpToDate Lexidrug, formerly Lexicomp.

Is the UpToDate drug interaction checker free?

No. The Drug Interactions tool sits behind an UpToDate login, so someone pays for it: your hospital, your university or you. UpToDate does not publish a personal subscription price; its store asks for your country and role and then a sign-in. If you only want the interaction database, the UpToDate Lexidrug app costs $29.99 a month after a one-month trial.

How do I use the drug interactions tool in UpToDate?

Open UpToDate, click Drug Interactions under the search bar (or in the menu at the top right), type each drug or herb to build the list, then click Analyze. Each interacting pair appears with its risk rating. Click a pair to read the interaction monograph, which gives a summary and patient management advice. Results can be filtered by rating and printed.

What do the UpToDate interaction ratings A, B, C, D and X mean?

They are Lexidrug risk ratings. A means no known interaction. B means no action needed. C means monitor therapy. D means consider therapy modification. X means avoid the combination. In practice D and X are the two that should stop you before signing, and C deserves a monitoring plan in frail or renally impaired patients.

Is the UpToDate interaction checker the same as Lexicomp?

It uses the same interaction database. Wolters Kluwer owns both products, renamed Lexicomp to UpToDate Lexidrug, and says the same editorial team develops the drug content in each. Lexidrug adds fuller drug monographs and extra modules around the checker; UpToDate wraps it in clinical topics.

Can I get the UpToDate drug interaction checker without a hospital login?

Yes, three ways. Buy a personal UpToDate subscription, whose app listing names drug reference and interactions among its features. Subscribe to the UpToDate Lexidrug app at $29.99 a month for the same interaction data. Or use a different checker: Medscape and the basic Epocrates check are free, and Prescriber.io screens the whole list against the patient for $24 per clinician a month billed annually.

What is the best alternative to the UpToDate drug interaction checker?

It depends on what you lost. For the same ratings and monographs, the Lexidrug app is the direct substitute at $29.99 a month. For a free pairwise check, Medscape works. If you want the list checked against the patient, with contraindications and renal or hepatic dosing in the same result, Prescriber.io does that for $24 per clinician a month billed annually.

Check the whole list against the patient before you sign

Interactions, contraindications and renal or hepatic dosing in one card, with a cited reason on every flag. You review, verify, and sign.
